Interface Pharmacists can support practices and primary care networks (PCNs) with their QOF 2023/2024 achievement. This will increase disease prevalence, clinical care and the associated income for practices.
Interface can identify patients attending the practice but missing from clinical registers. These patients are often being treated, but are missing from QOF disease registers due to coding errors or omissions. In turn, practices are not receiving the correct level of funding. Without proper register maintenance, patient care may also be impacted with diabetic patients potentially missing annual reviews for example. Interface take things further, offering two forms of support in addition to the patient management platform itself. Our Clinical Pharmacists work alongside practice and PCN staff to implement the report, training them on the niches of QOF and patient addition while also being able to add patients directly onto practice registers in set domains.

Our Clinical Pharmacists have developed a bespoke set of searches to support practices and PCNs with their QOF achievement. On average, we identify £1,900 of additional income per 1,000 list size.

They can produce a free Health Check to give you an insight into your current QOF standing. The team can also run through patient examples, discussing their inclusion and appropriate next steps as well as a live demonstration of the QOF Enhancement Tool so you’ll know how the service works. When your order has been processed, you’ll receive your invoice automatically via email. When the payment has cleared, the team will get in touch to arrange the delivery of the service. Depending on the Tier you’ve chosen, the team will arrange your consultancy day/s and arrange for your tool to be produced. If you have opted to go for Tier 1, the report will be produced as quickly as possible so you can get started immediately.
If you’ve opted for a Tier 2 or above, Consultancy Days are included in your service. These will be delivered by one of our leading Clinical Pharmacists. They will get in touch ahead of your agreed delivery date to introduce themselves, arrange a timetable for the day/s and discuss your priorities for the service.
Our Clinical Pharmacist will attend to deliver your Consultancy Day/s. They will talk you through your tool and help you implement your practice plan. Working with your staff, the Pharmacist will begin sorting through the identified patients, discussing their inclusion, reasons for their current omission from registers and next steps to move them on to your registers. Coding support is also available as part of our Consultancy Days. Our Clinical Pharmacists can work through the QOF domains and begin coding patients themselves helping practices, PCNs and ICSs to rapidly work their platform and begin managing the backlog of patients resulting from COVID-19.
As part of our QOF Disease Prevalence Service, we offer year long support irrelevant of the Tier you chose. All practices will be able to request two Revalidation reports to check progress. Your practice team will also be able to call the Service Support team with any QOF questions or difficulties with individual cases or using the tool itself. If you’ve opted for a Tier 2 or Tier 3 service, you also have a structured Telephone Conference session included to provide additional support. This hour long session can be booked at any time within the QOF year.

Interface has developed the Heart Failure (HF) Plus service to support Practices and Federations to investigate their heart failure registers, with the help of an experienced clinical pharmacist.
The Heart Failure Plus service is an additional module of our QOF Enhancement Service. In delivering QOF support to over 1,000 practices we recognised a need for further support within HF.
Practices can purchase additional support from our team of QOF trained technicians based in our Head Office.
