Pharmacist-led reviews to proactively manage patients with COPD and overcome the backlog of care

Interface Clinical Services, an IQVIA business presented a poster at the Primary Care Respiratory Society (PCRS) with NHS Leicester, Leicestershire and Rutland Integrated Care Board (LLR). The poster detailed a programme of work that utilised Pharmacists to proactively manage patients with COPD and support the NHS to overcome the backlog of care.

The primary objective was to create a programme that would reduce the backlog of care and provide efficient care to high risk COPD patients and to reduce any clinical complications or unplanned admissions. The seven-week support service provided additional clinical pharmacist resource to assess and proactively manage patients with COPD, this included:

    • identifying gaps in care to recognise patients at risk of adverse events
    • Providing skilled capacity to deliver best care at scale and address healthcare inequalities
    • providing mentorship opportunities to the multidisciplinary team to sustain enhanced care.
