      Through comprehensive clinical notes review and face-to-face* pharmacist-led clinics, Interface pharmacists can clinically assess individual patients and make recommendations for management or referral to specialist services.

      Interface can support pharmacists to:

      • Optimise and rationalise diabetes management by aligning to best practice guidance and/or local management pathway
      • Improve achievement of the NICE-recommended 9 key care processes for diabetes through provision of an audit tool
      • Reduce practice burden of type 2 diabetes management
      • Provide patient education relevant to self-management, diet and lifestyle

      Interface can provide the practice with a report to demonstrate Quality Improvement in the management of patients with diabetes to support future workstreams and PDSA cycles.

      This was a timely service which we found benefitted our patients greatly. The data collection was very rigorous and evidence based and identified all the patients appropriately. Patients were then all consulted with on a pre-arranged basis. They were given clear and detailed explanation of the programme and its intention. The Interface pharmacist had a very detailed knowledge of the subject and had a good working knowledge of the issues involved. We received detailed reports on each case which were all also discussed with the lead GP. We were very happy with the service.
