QOF Disease Prevalence Coding Support

Now for the fourth year running, Interface Clinical Services have been accepted to present at the RCGP Conference.
Taking place in Liverpool between the 24th and 26th of October, Interface Clinical Services will present a poster to discuss our work with the Modality Partnership to support in the achievement of the key long-term goals of COPD management.
The initial baselines analysis identified 0ver 1,600 COPD patients across 10 GP practices. Using the defined criteria, 604 patients were prioritised by the MTD for deeper review. Subsequently, 276 patients were subsequently seen in a pharmacist led clinic.
In total, over 1,000 actions were implemented working with the 10 practices to support in patient care. These interventions were comprised of both pharmacological and non-pharmacological interventions. Amongst the other interventions recorded, patients received inhaler technique training, provision of spacer devices, referral to pulmonary rehab or smoking cessation, FEV1 and pulse oximetry through comprehensive face to face support.
