Using the practice clinical system and specially designed searches Interface can:

• Identify cohorts of patients for review, including patients who should be testing and are not, and those who are at risk of poor diabetes control due to sub-optimal testing patterns
• Ensure that patients are aligned to practice-preferred meters and test strips
• Highlight where patients with diabetes are missing any of their recommended key care checks
• Prioritise workloads and help to improve patient care whilst improving QOF achievement
• To improve patient education and understanding of blood glucose management through pharmacist-led clinics

Interface can provide educational support to practice staff, and practices will receive a report to demonstrate Quality Improvement in the management of diabetes through self-monitoring of blood glucose.

The pharmacist was brilliantly organised and dealt with everything from identifying and writing to the patients offering them an appointment with him, whilst keeping the Practice informed with documentation all the way through the process. All we had to do was actually put his clinics on the screen. Patients who did not take up the offer were further written to at the end of the process requesting that they speak to the nurse at their next nurse review. He left us with a full audit, including the projected cost savings to the Practice.
