Lipid Management

With cardiovascular disease (CVD) responsible for 1 in 4 deaths in the UK, lipid management for secondary prevention of atherosclerotic cardiovascular disease (ASCVD) is a clinical priority for patients and the NHS. Interface can proactively support primary care with their lipid management.

Patients requiring lipid optimisation for secondary prevention of Atherosclerotic Cardiovascular Disease (ASCVD) can be identified and reviewed, preventing further cardiovascular events including heart attacks and strokes.

The service can support practices to optimise their clinical management in line with national and local guidelines, and help achieve their QOF indicators.

• Stratify patients with a diagnosis of ASCVD and report on their current management,  identifying patients who may benefit from review and treatment optimisation.
• Provide clinical capacity to deliver patient-facing or virtual clinics.

• Provide mentorship opportunities for practice teams to support sustained quality improvement in lipid management.  
• Support the achievement of CHOL001 and CHOL002 QOF indicators.
