We have been working with an anticoagulant support pharmacist from Interface Clinical Services (ICS) with the aim to optimise anticoagulant therapy and reduce stroke risk in patients with atrial fibrillation.
The ICS pharmacist who managed the project identified the following groups of patients with AF:
• Currently not anticoagulated with a moderate – high stroke risk (male patients with a CHA2DS2-VASc ≥ 1 or female patients with a CHA2DS2-VASc ≥2)
• Currently poorly controlled on warfarin; with a time in therapeutic range calculated as below 65% over at least the last six months
One of the main benefits of the service is that the ICS pharmacist then led clinics with appropriate patients to discuss their anticoagulation options; be it initiation of or transition to Direct Oral Anticoagulant (DOAC) therapy. Prescribing decisions were fully under my control and authorisation and carried out with optimum safety. The ICS pharmacist provided full counselling and education to each patient.
The ICS pharmacist became an integrated member of the team, with all our GPs referring appropriate patients to her for review and our lesser experienced GPs using her as a point of contact for DOAC queries. The amount of clinician time this has saved for us has been of great benefit to the practice. I would be very happy to recommend this service to any GP practice who is keen to ensure that their AF patients are optimally managed.

The Interface pharmacist was very efficient at highlighting patients who were not coded correctly and at increasing the accuracy of our problem lists. The workload following this was modest as patients were being managed correctly but simply hadn’t got the appropriate code attached. This has increased our accuracy and prevalence significantly.  The pharmacist carried out her own surgeries further minimising the workload on practice staff and clinicians.

I would like to advise that we have been dealing with Interface Clinical Services since the start of 2017.  In that time, the pharmacist has been here on several occasions, firstly to look at our Qof figures and particularly our prevalence. Through the work that she has done, we earnt more money for QOF than we had anticipated, mainly due to not having enough patients on our disease registers and the missing patients being identified and added to the appropriate registers they should have been on all along.
Apart from QOF, the pharmacist has been with us for Audits and Reviews for our Atrial Fibrillation patients, our Diabetics and is currently working on our Asthma patients – reviewing their medication, giving them advice, and support.  All the patients are very pleased with the service they have been given. 
Also, while remaining in contact with and discussing appropriate clinical governance with our lead GP, the impact on his time and that of the admin team has been minimal.
Getting Interface Involved with our service would be classed as a big win.
