COPD

COPD is the second largest cause of emergency hospital admissions in the UK.
Each year, approximately 1 million bed days are accounted for by COPD patients.

Through our Clinical Pharmacists, Interface can support in the systematic management of patients with COPD to reduce symptoms and the risk of exacerbations and therefore referrals on to secondary care.

      Using your practice clinical system and specially designed searches Interface can:

      • Reduce symptoms and exacerbations in high-risk patients through pharmacological and non-pharmacological recommendations
      • Rationalise the therapeutic management of stable COPD patents
      • Reduce practice burden of long-term condition management through provision of Clinical Pharmacist-led patient clinics

      The pharmacist will identify COPD patients with persistent symptoms but not showing significant exacerbations. These patients can then be reviewed and action plans produced to manage their treatment going forward.

      Patients with minimal symptoms & significant exacerbations, and COPD patients with persistent symptoms & significant exacerbations, will also be reviewed against national guidelines.

      Those with a hospital admission managed outside of local guidelines will be identified and flagged to the practice for further review. Detailed dashboards are presented at baseline and on completion to support Quality Improvement initiatives and PDSA cycles. Our therapy reviews can be funded at no additional cost to the practice, as we work in partnership with practices and PCNs, to help manage the additional burden as a result of the COVID pandemic.

      We had a COPD clinic run by Interface. It was very well attended by our patients and feedback from patients was really good. It also enabled our nurses and also medical student to sit in on the clinics and learn more about COPD and reviewing patients. These clinics also helped us with our QOF and we were also offered a free QOF health check which was very useful to our practice. Although we are a small practice we found it to be very beneficial. The pharmacists that did the clinics were very professional, knew what they were doing and worked along side the team in the practice with no disruption. We would definitely run clinics again with Interface
